How to Message Us on Elation Passport

A quick, friendly guide from your Seven Hills Family Medicine team


At Seven Hills Family Medicine, we love staying connected with you—and Elation Passport helps us do just that. This secure online tool lets you message us directly, view test results, request refills, and more.


But here’s the thing: we’ve noticed that some messages are being routed to the wrong people, which can delay our response to your needs. Let’s walk through exactly how to use Elation Passport to get your message to the right place—quickly and safely.



💬 Getting Started: Sending a New Message


When you log in to Elation Passport, you’ll land on your Messages screen, where you can view any current or past conversations with our team.


To send a new message:


  1. Look to the top right corner of the screen.


  2. Click the pen-and-paper icon ✏️📄 — this opens up a new message.


You’ll then be asked to select a "Reason" for your message. This is important because it determines who receives your message.



📌 Choose the Right “Reason” for Your Message


Here’s how messages are routed based on the reason you select:


  • Appointments → goes to our Care Coordinator. Use this for scheduling issues/questions and questions about your membership.


  • General Inquiries, Referrals, and Prescriptions & Refills → these go to your team nurse. We’ll help with refills, general questions, and next steps.


  • Questions About My Health and Lab or Other Test Results → these go directly to your physician who can give you further medical advice.


Choosing the right category ensures your message gets to the right person without delay!



🔒 A Note About Privacy: Use Elation, Not Email


Elation Passport is HIPAA compliant, meaning it protects your privacy and medical information. Email is not HIPAA compliant, so please do not email us with personal or health-related questions. We pride ourselves in protecting your privacy.


Elation is the best way to get in touch with us for anything medical. We check it regularly, and it helps us respond to your needs safely and efficiently.
