How to Set Up Elation Passport: Your Guide to Secure, Convenient Patient Communication

Ready to message your care team, see your visit notes, and manage your health in one easy place? Meet Elation Passport—our secure patient portal, designed to make communication simple and stress-free.


Here’s how to get started:



1. Create Your Account


After your first appointment, you’ll get an email invite from noreply@elationpassport.com.

  • Click the link in the email

  • Choose a strong password

  • Save your login info somewhere safe (we recommend a password manager!)

Haven’t seen the invite? 

Check your spam folder, or email us at info@sevenhillsfamilymedicine.com and we’ll resend it.


🔒 Note: The activation link expires after 24 hours. Contact our office if you need a new link.



2. Log In from Any Device


You can use Elation Passport on your phone, tablet, or computer—whatever works best for you.

  • Use the mobile-friendly site: Go to www.elationpassport.com in any browser

  • Or download the app: Search for "Elation Passport" in the iOS App Store (iPhone only for now)

3. Use Passport to Stay in the Loop


Once you have logged on, you can:

  • Send and receive messages from your care team

  • View past visit notes

  • See your lab results (once they’re reviewed)

  • Upload documents or share information before a visit

4. Skip the Phone Tag—Use Passport Instead


We check Passport messages every weekday. It’s the best way to:

  • Request refills

  • Ask a quick question

  • Share a health update

  • Follow up after your visit

  • Book an appointment

Frequently Asked Questions (FAQ)


❓ How do I view my upcoming appointment?

  1. Log into Elation Passport

  2. Click the Clinical Profile tab

  3. Your upcoming appointment day and time will be displayed on the upper right




❓ What about appointment scheduling?


You can schedule your appointments through the Elation Passport portal — just click the Booking button on your dashboard. This takes you directly to our official booking page.



A few important reminders:


  • Always schedule with your assigned physician, unless we’ve told you otherwise.


  • Nurse-only visits — like STI screening (without symptoms), lab draws, self-injection training, Depo shots, and blood pressure checks — should be scheduled with the nurse.


⚠️ Don’t use the “Guest” option on the scheduling page.That creates a duplicate chart, which means you won’t be able to see your upcoming appointment or access your health info. Always log in when booking.


❓ Can I cancel an appointment through Elation Passport?


Currently, appointments cannot be cancelled directly through Passport.


To cancel an appointment:

  • Use the appointment confirmation email

  • Confirmation emails are sent automatically 3 days, 1 day, and 1 hour before your scheduled appointment

  • Click the "Cancel Appointment" button or link in that email



To reschedule an appointment, follow the steps above, then schedule a new appointment for your preferred time and day. If you can’t find the confirmation email or need assistance with scheduling, rescheduling or canceling an appointment, please contact our office!



❓ I didn’t receive a confirmation email. What should I do?


  • Check your spam or junk folder


  • Make sure we have the correct email address on file


  • Contact our office if you need to update your contact info or need a new confirmation email sent



❓ Is my information safe in Elation Passport? 


Yes—Elation Passport is fully HIPAA-compliant, ensuring that your medical information is securely protected.



❓ Can I update my contact information ?


  • From your Elation Passport profile, click on your name in the top right corner and select Settings from the drop down menu


  • Set your preferred contact method, opt in to text updates, change your email or password


  • Contact our office if you cannot access your Passport account and we’d be happy to assist!
